What is the CCK Patient Portal? CCK’s electronic medical record system connects to a patient portal called Ontada Health. This is a secure site that lets you view parts of your medical record and stay connected with your CCK care team—anytime, from your phone, tablet, or computer.

Please note portal messages are reviewed and answered during business hours, only.
The patient portal account must be requested and created by the patient first using the patient’s own email address. If you are a caregiver, spouse, or involved family member, you cannot sign up with your email on the patient’s behalf.
Once the patient has successfully registered and logged into their portal, they can choose to add you as a caregiver and control what access you receive. You will then have your own account created for the patient’s care.
